Recognition Award

Dear Teacher/Administrator/Staff: 

Please complete each of the following requirements by Tuesday, April 15, 2025. If any portion is omitted or postmarked after the deadline, your application will not be considered. 

CAD Senior Recognition Award in consideration Checklist: 

Part A: Complete the personal information, education, future plans, and accomplishment sections.

Part B: Complete the ASL video nomination 

Part C: Complete the written nomination (optional if you are camera shy)

Part D: Have two instructors/coaches/community members write letters of Recommendation.

Part E: Sign and upload the letter of agreement.
