Sponsorship

Sponsorship requests are reviewed prior to each regularly scheduled Executive Board meeting. After the review process, the CAD Secretary will notify you of the decision regarding your request.

Please attach any supporting documents, such as letters, invoices, or additional information, at the end of this form.
