This application is to be completed by teachers or administrators of the students. The students are not to be made aware of their nomination for this award until after CAD board selects the awardees. It is CAD’s goal to grant awards to 3 students with one from northern California, one from central California and one from southern California.
Each student awardee will receive a CAD medal to recognize and celebrate their achievement. Student awardees may wish to wear the medal during their graduation ceremony if permitted by their school.
CAD Award Eligibility
• Deaf/hard of hearing student
• Graduating senior
• Overcoming challenge(s) during or throughout student’s educational career
• Maintains positive outlook and attitude
